Botframework-composer: Ability to hand off from bot to a human agent via Composer

We need the ability for our bot, developed with Composer, to be able to hand off to a human operator.

Bot Framework has documented how to transition to a human. It would be great for Composer to implement hand-off in a consistent manner.

I have found two closed issues relating to hand off - #3077 and #3624. There is a link to a "custom action topic" in #3624 which is broken.

I think this is something that is needed as this is a pretty crucial function for a real world bot.

I have also checked the Composer docs and there is nothing mentioned about hand off there at all.
