Sorry you're having trouble! You've hit all the usual debugging advice I'd give you (check the hardware config is correct. Check your collaborators have the audio extension. ...). So sounds like this may be a proper bug!
@jramsay should be able to help you out.
collaborators did have the extension :) we all were trying to mute and unmute ourselves to see if we could magically toggle it active. Thank you for the quick reply!

I have a similar problem - but only with me, second person in session can speak and I hear him, but he doesn't hear me. And in addition, I don't see VSCode in mac privacy settings, so it probably cannot get access to microphone, but there are no errors in logs, no pop-up to allow microphone or so.
Just tested with 10 students. I am on pc. Other pc users had no issue with audio. Only issue when students were on mac.
I also have this issue on macOS Catalina. The extension needs to prompt macOS users for microphone permissions. Users would then grant access in macOS system preferences.
We just released an update to the audio extension (v0.1.91) to the VS Code Marketplace that should address this issue. I'm going to close this and related items as fixed. Can you give it a go and let us know if it works for you? Thanks for all your help with this issue!
