Macpass: Windows associations not saved with a kbd file

Hi,

Just to let you know, that windows associations is not saved with a kdb file.
It works well if it is a kdbx file.

Autotype features aren't supported at all on KDB files. I neither parse the special notes nor do I save them. I even considered dumping support for KDB completely since it's less code to maintain.
