Live-share: Allow hosts to share abitrary files with guests outside of the folder/solution root

In addition to multi-root support (#46) we could allow hosts to share arbitrary files with guests that they have opened up in their editor. (Also related to #57 - sharing unsaved files.)

This feature is now available in the latest update for VS and VS Code 馃帀 If a host opens a file that it located outside of the shared directory/solution, then any guest that is following them will automatically see the contents of that file and can co-edit it with them. Let us know if you run into any problems with this experience and/or have feedback on it. Thanks!
