Slack: Post message to Slack when PR changes from Draft to Ready for review

With the introduction of Draft PRs and the shipping of https://github.com/integrations/slack/pull/791, it would be great if we could be notified in Slack when a PR's status changes from Draft to Ready for review.

Something like this would be nice:

Pull request status changed from Draft to Ready for review by dbuskariol
Reviews requested for team-name
