Mattermost-server: Update help text in Leave Team modal

Users are accidentally hitting the "Leave Team" button when they intend to leave a channel.

Proposed fix: Update the help text in the "Leave Team" modal to:

**You will be removed from all {num_of_public_channels} public channels and {num_of_private_channels} private channels on this team**. If the team is private you will not be able to rejoin the team without an invitation. Are you sure?

Where

• the first sentence is bolded (marked by **),
• {num_of_public_channels} refers to the number of public channels that the user is a member of in the team, and
• {num_of_private_channels} refers to the number of private channels that the user is a member of in the team, and

I tried my best to setup a development environment. But my PC just did not support it. I'm extremely sorry.
I got on the mattermost help channel as well. Didn't turn out to be of any use. I kept at it for over 5 weeks and finally gave up man.

Hey i would like to work on this if any help is needed.
Quick clarification, is the issue about replacing the "XX" mentioned in the question with respective team names?
Because otherwise the message displayed while leaving a team seems to be quite clear
