Since it relies on iTunes Wi-Fi sync, you can't just point AltStore to an external AltServer (not even via VPN). One possible solution would be to host a web server on the AltServer where apps are served. I'm not sure if it's possible to implement though.
It鈥檚 definitely possible. However, there is a 100-device limit for ad-hoc provisioning profiles, so you鈥檙e going to have to do it yourself. If that limit didn鈥檛 exist, I believe everyone could use a server @rileytestut hosted, without any macOS or Windows client. Assuming he could host it, of course.
Multiple account鈥檚 on AltServer is already possible. So a single server, multiple accounts is possible already. Obviously, there needs to be some code changes to make AltServer / the phone discoverable via VPN.

Would be cool, I have site to sites setup so I can run it off-site and just enter the IP of that machine.
The primary issue is the security risks involved because the user's Apple ID and password would have to pass through a public server and that takes on additional risks.
Additionally, most servers run on linux based architecture and AltServer does not work on Linux yet.
Despite all the roadblocks, I'd say it's not an impossible feature, but it's also not likely for the foreseeable future. If someone else manages to get it working, it could potentially get merged into master.
The actual problem that makes it impossible is that iTunes Wi-Fi Sync only works over local network and VPN isn't supported; it also requires mDNS for Bonjour. Apple ID privacy isn't the issue as you can just create a throwaway account.

I'm working on this. There is the L2TP route, but after 6 hours I gave up on the protocol. I have a couple more ideas and I'll update you guys when I get my AltStore Anywhere solution working. 馃

What have you tried. I am testing it with friends. Are you using softether? because that would more likely emulate a full Ethernet protocol Wich internet uses. We thought of using l2tp over IPSec for maybe a two months or three on research but some of us can't port forward. It seem promising so far but we need to learn more abut setting up l2tp.
We have someone that's trying but they did same thing they gave up because of some bug or user error. But will come back and try again.
