Web: Consistent Form Styles Across the Gitcoin

As a user, I want to see legible and consistent form styles across the app so that I know where to input information, get things done, and build trust with the Gitcoin brand.

Description

Unify form styles on the following pages:

• Anywhere forms exist in the app portion of the site.
• Anywhere forms exist on marketing pages: slack sign up, email settings, etc.

Assumptions

Keep current functionality of each form

Definition of Done

All forms are using the same form design pattern
