Nope. It shows Waterfox.app as being the default there, but still each
time the application is terminated and restarted, it will ask the same
question again...

Is this issue reproducible with Waterfox 56.0.3 on Mac OS X?

(I don't recall finding the issue with an earlier version of 56.x.)

This is resolved in the newest versions of Waterfox for Mac. The only issues that persist are for Windows, due to registry settings. Since Mac doesn't use registry settings, this should no longer be happening.

I am on OS X and it's working without issue. Please re-open if you still have the issue.

Sorry, have not been able to test this, I had to evacuate twice in a
month, first back in early December because of the #CreekFire, and then
last week because of possible mudslides in the aftermath (as in
Montecito a bit further up north) and just hooked my Mac back up last
night.

I will check it out as soon as I get some breathing room, currently in
"catch up" mode after all the recent stress...

After getting finally back up to normal after the fire and the
subsequent mudslide threat, I was just able to update Waterfox on my
MacBook Pro (after it took a while to run the updates to macOS
10.13.3) and I can confirm that with the latest version of Waterfox
(56.03), it is now properly able to set itself as the default browser...
