Personally, i don't like the fields being glued together. It would be no problem to seperate those into individual input fields. See my examples below.

We use the awesome https://github.com/richardvenneman/floatl for an application of ours (Website, Example usage). It works great but we believe it to be incompatible for semantic UI (out of the box). But it might be a good place to look for implementation-inspiration.
I'm a fan of this implementation - https://geminilabs.github.io/float-labels.js/ - is light. Would love this to be a component of semantic.
