Note: The screenshot in the bug has the NONCE column instead of the ID that's in the later code. Maybe this is an old bug, already solved with most recent changes?

Also wasn't able to reproduce this, not even on production, where this bug was encounctered.
Seems like this happened while using Pillar Wallet through WalletConnect.

Got some more information:
Safe was created some time ago (beginning of november).
The wallet used was Pillar, but it was an imported seed phrase, not a EOA created using Pillar. (Shouldn't matter)
This is very likely connected with #352
