Magento2: issue is existing user login at checkout produce error - "Provided form does not exist"
New Magento 2.0.2 install, creating user works fine from main / home page and link, however if the existing user add product to cart and attempt log in to their account upon checkout does not work report error: "Provided form does not exist", see attached screen shots.
Any advise hot that could be fix?

The issue was turned On CAPTCHA, as soon I turned OFF all problems with existing user checkout login disappeared. Everything start to work just fine. So do not turn CAPTCHA for customers ON, not for guest checkout or otherwise.
