This is discussed in #1867, there are some more workarounds in that thread. It's due to the new security controls that 10.15 introduced, and Karabiner is not requesting the whitelist status.
I was able to get my instance to work by adding the observer / grabber applications to the security system preferences, ending all of the services, and starting again.

Unfortunately, after a restart I'm back at square one. Thinking there might be a script that could ad-hoc resolve this until there's a legitimate fix?

@natefromfargo Thanks for pointing out the thread. I tried to follow the solution but I can't locate org.pqrs in my drive. Is there anything wrong with my installation? I reinstalled it.

For anyone else having trouble with Karabiner-Elements, I've got it working pretty consistently even with the aforementioned problems. I've added karabiner_kextd along with the karabiner_grabber and karabiner_observer to the security list.

Here's a gist with a shell script I made to automate killing karabiner and restarting it, it's pretty simple:

Save the above somewhere and chmod +x then run it ./karabiner-fix.sh when Karabiner stops working.

P.S. You'll very likely have to run this each time your mac wakes up from sleep.

@tehprofessor a simple sudo killall karabiner_observer karabiner_grabber also works, without needing to relaunch the host process (it auto restarts these child processes)
