If you fall into shallow water, you should take fall damage as you would if you regularly fell that distance. Only if you fall into deep, swimmable water, will you not take any fall damage.
Or at least some knockback/damage. Right now, is a bit too 'unreal'..

Yes, I thought about this (popular method in zombie escape), that's why I recommended at least a knockback (or screen effect like HS) for a little realism :)

Yeah, I would say keep gameplay mechanics the same as to not break any mods or custom maps. If it's working as intentional and not an exploit, there's no reason to change it.
I agree that integrating this into the game proper is too big of a change to gameplay mechanics. Having this available as a mod is a perfectly fine way to enable it.
