Running exa 0.9.0 on macOS Catalina 尾4 in the stock Terminal.app with Bash 5.0.x. Seeing this:

Yes, you need to have a font installed that has the icons themselves. I recommend checking out nerd-fonts. Here's what mine looks like with the Hack nerd font in use:

Got it. Installed, and now everything is working. I'll resolve the ticket, but it would be nice if this were more clearly documented.
