Can't open some doors, as they don't appear as active objects (although they are visible on the map).
Sometimes there's active doors in the middle of nowhere (usually close to the place where they should be) that are active.
The save I started from was from the PC version of the game.

What version were you using? Could you let us know what floor you were on.
By PC version do you mean vanilla Diablo 1.09?
Compiled today from master. I played the PC version ~10 years ago so the answer is : probably ;)
As for the level this started in the level entered via caves (9?) and the one below.. Attached saves are directly next to occurrences of this bug.
When I went back to 1.0.1 the map was drawn differently and the doors were accessible on level 9 (there were no stray doors anymore too) but not on level 10 (where the doors were in a different places, but were still not actionable).
I've rebuilt from git master today (32778cdbb9482ba527014325e0c04d9a50c8e632) and the issue was the same as yesterday. I'm attaching todays build for reference.

7f7ab8c3dd7ba53264c9ba295513c7020704234f still has the issue, produces same map layout as 1.0.1 (different from the latest in master)
Thank you for helping to identify this issue, it has now been corrected upstream and merged.
Note that you save game maybe have been corrupted from playing the buggy version and saving the incorrectly generated lvl 10
