Book: Update the book for procedural macros
This issue tracks inclusion of information about procedural macros into the Book. This will happen in the 2018 edition.

I’m working on this!
In general, I cannot accept entire chapters written by other people for legal reasons. I appreciate it though! I hope to open the PR Monday and will cc you for a review :)
