Here is explicitly defined that the form-recognizer is not publicly available.
If there is an publicly available image for the form-recognizer the docker pull commands are out of date:
docker pull containerpreview.azurecr.io/microsoft/cognitive-services-form-recognizer:latest
i don't know where to look for the public available cognitive-services-form-recognizer.
docker pull azurecr.io/microsoft/cognitive-services-form-recognizer:latest results in time out errors
mcr.microsoft.com/azure-cognitive-services/form-recognizer:latest
and variants are not working.

You're seeing the mcr.microsoft.com/azure-cognitive-services/custom-form/labeltool
container in the MCR. This is not actually the Form Recognizer container, but rather "Sample Labeling Tool" container. This is different. #please-close

here the documentation is describing the way how to use the 'Form Recognizer Container'. This documentation is now obsolete/out of date?

I'm also stuck on the directions as I cannot pull the docker container image without username/password for the registry which the directions claim we will get after filling out the request form (which is no longer required).

I apologize for the confusion... I'm working to re-open this issue, I was just informed that the product team mistakenly decommissioned the request form in the excitement to have the Form Recognizer service offering being announced as GA -- with the oversight that the container is still in gated preview. I do apologize for this inconvenience. We will update the doc and have an updated link to a new form to request access early next week. Thank you

@IEvangelist perhaps someone can correct me if I am missing something here- but when I click the link in the Container Registry section to access the request form (https://aka.ms/FormRecognizerRequestAccess) I still get:
Form Recognizer is now publicly available. To get started please go to the Azure Portal to create a Form Recognizer resource. Follow one of the Quickstarts located here https://aka.ms/form-recognizer/docs to extract text, key value pairs and tables from your forms.
This is the link that you should use: https://aka.ms/FormRecognizerContainerRequestAccess, there was an inconsistency with URLs in the docs -- that is being updated.
