If I use premium performance it appears to not be creating the $web container but if i choose standard performance on the storage account its working. Anyone else seeing this issue ?

This is by design. Premium storage is intended for VM disks. https://docs.microsoft.com/en-us/azure/virtual-machines/windows/premium-storage
OK maybe it should be disabled on the portal if its selected than to reduce confusion.
I agree. I'll pull in the right folks to get this fixed. You can leave the issue open for now.
Azure CDN endpoints can only point to premium storage, correct? Still no way to get a static app behind the CDN for URL rewriting and proper SPA hosting?
That is not correct, no. CDN supports standard storage. You can use CDN with static websites.
